Each NHS number is made up of 10 numerical numbers, detailed in the 3-3-4 format, as follows:

Should you have an old style Medical Card, it will detail the old style NHS number made up of both numbers and letters, which has now been replaced the by the new style made up entirely numbers.

| Currently Registered with a GP Practice: |
| | Contact your GP Practice and ask them to look it up for you, you may be asked to provide proof of identity, to protect your privacy and security |

| Not currently Registered with a GP Practice: |
| | Simply ask your Primary Care Trust to look it up for you
Your local Primary Care Trust can be found at www.nhs.uk and go to Primary Care Trusts in the Find Services section and enter your postcode. Alternatively call NHS Direct on 0845 4647
When you telephone your local Primary Care Trust ask for the department dealing with 'Patient GP Registrations' |
